Eligibility

If you are a full-time employee regularly working an average of 30+ hours per week, you are eligible to participate in the medical, dental, and vision benefits program. Employees regularly working an average of 35+ hours per week are eligible for Life and Disability coverage.

 

Benefits begin on the first of the month following your date of hire. You have the option to enroll yourself and your eligible dependents in Ilera Holistic Healthcare's benefits program.

 

Medical, vision, and dental coverage will end at the end of the month following date of termination or resignation.

 

Life insurance, short-term and long-term disability coverage will end on the date of termination or resignation.

If you don’t enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.​

Coverage will end at the end of the month following date of termination or resignation.

Eligible dependents include:

• Your legal spouse. 

• Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.
