Life & Disability Insurance
Life and disability benefits are provided through UnitedHealthcare.
Basic Life Insurance & AD&D
Life Insurance provides additional financial protection for you and your family in the event of your death. Ilera Holistic Healthcare provides life insurance of $25,000. There is no cost for this coverage.
Supplemental Life and AD&D
If you determine you need more than the basic coverage, you may purchase additional coverage for yourself and your eligible family members.
Employee Coverage

Choose from $10,000 to $100,000, in $10,000 increments.

Benefit is employee-elected at enrollment.

Amounts over $30,000 require Evidence of Insurability.

Coverage reduces to 65% at age 65 and 50% at age 70 and ends at retirement.
Spouse Coverage

Choose from $10,000 or $20,000.

Must elect employee coverage to cover a spouse.
Child Coverage

Choose from $5,000 or $10,000 (per child).

Covers children from birth up to age 26.

Must choose employee coverage.
Note: You must enroll within 31 days of eligibility to avoid Evidence of Insurability. Late enrollment or increases in coverage may require additional approval.
Disability Insurance
Disability insurance provides benefits that replace part of your lost income when you become unable to work due to a covered injury or illness. The short-term disability benefit is 60% of your weekly income up to a maximum of $1,500 per week. This benefit begins after the 7th day of disability and is no cost to you.
The long-term disability benefit is 60% of your monthly income, up to a maximum of $6,000 per month. There is no cost for this coverage. This benefit begins after the 90th day of disability.
